If you have been banned from visiting Valley View but complete this registration and submit payment, you WILL NOT be admitted and you WILL NOT be refunded. If you are not sure or suspect you have been asked not to return, please call the office at 608-770-8872 or email our office where you can confirm your eligibility status.
